I'm filing for A&A for my mother. She is in a assisted living facility so i answered no to the question that ask if she was in a nursing home in the 21-534 form. Do i need to send anything from the assisted living facility as proof of residence and cost?

YES TO YOUR QUESTION. SEND A COPY OF AN ITEMIZED MONTHLY STATEMENT, AS WELL AS THE NURSES ASSESSMENT LOGS, (OR ANY
ADMITTING MEDICAL DOCUMENTATION WITH DIAGNOSIS.) i DO NOT KNOW THE STATE SHE RESIDES IN, BUT YOU ALSO NEED TO SUPPLY THE VA WITH ANY AND ALL UPDATED MEDICAL DOCUMENTATION THAT YOU CAN PROVIDE TO SUPPORT YOUR CLAIM FOR A&A.
BE SURE TO WRITE IN THE VETS NAME, YOUR MOM'S NAME, SS OR CLAIM # OF THE VET, OR ONE THAT HAS BEEN ASSIGNED TO YOUR MOM'S CLAIM. ALL OF THIS INFO SHOULD BE IN THE LOWER RIGHT HAND OF EACH PAGE SUPPLIED TO THE VA. (PRE-PRINTED LABELS ARE LIFESAVERS IN SUPPLYING ADDITIONAL INFO TO THE VA.)

in general, VA needs to verify that the claimant is actually a resident of the facility, the date admitted, the type of care they receive and what they pay every month to the facility for room/board/medical.

a statement on the assisted living facilities letter head is just fine.  a person from the buisness office can type that up for you.  be sure it is signed and dated with a contact number from the facility.

and the gal above is correct  - if the claimant is in an ALF, they need to provide medical evidence (SIGNED BY AN MD) showing that they are in need of assistance with their ADL's etc. what their diaganosis is, etc.

if an LPN signs or a PA signs, VA needs the doctor to co-sign the documentation.  Be sure you review the medical evidence before it is submitted and review WHO signed it.
